Home Health RN Case Manager

Daily Responsibilities: * Provide Nursing Services to patients in their homes * Complete 5 visits per day * Schedule your visits closest to home and start from home * Willingness to commute a 50-mile radius from home location if needed Requirements: * Licensed RN in the state of Virginia * One (1) year experience as a RN (Home Health, Med-Surg, ED, SNF) * Experience with IV’s, Wound Care, and Disease Management * Must be a licensed driver.
Ability to travel to all business locations.

* Current state license as a Registered Nurse.
* Proof of current CPR.
* Current driver’s license.
* Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice or equivalent environment.
