PROGRAM MANAGER – Group Home JOB DESCRIPTION
GENERAL:
Under the direct supervision of the Program Director and accountable to the Chief Operations Officer, The *PROGRAM MANAGER *assists individuals in the development of adult basic living skills to achieve maximum independence and ensures that all written work meets all applicable licensing standards.
The overall objective of this position is to support the individual needs of people with Intellectual and Developmental Disabilities served in Residential Group Homes.
The services provided should result in safe, normal living situations for individuals where self-direction, integration into the community and effective implementation of Person-Centered Principles *SPECIFIC:* The PROGRAM MANAGER will: Administrative: * Monitor the group home according to the policies and procedures established by the Angelic Place Board of Directors and Administration.
* Demonstrate sound fiscal management of individual accounts in accordance with established Angelic Place spending guidelines and procedures.
* Notify the Program Director or Chief Operations Officer of household and maintenance needs.
* Provide for the safety and security of individuals, staff, and property.
* Encourage community awareness of the needs and capabilities of people with disabilities and individual programs.
* Assist with orientation and supervision of volunteers and all staff.
* Protect the human rights of individuals and safeguard confidentiality.
* Participate in staff meetings, in-service training sessions, and *assist as needed*.
* Establish and maintain appropriate relationships with staff, individuals, families, and other agencies and individuals dealing with the program.
* Use and further develop personal qualities such as flexibility, common sense, responsibility, problem solving, communication, initiative, and sense of humor.
* *Monitor data and progress on the individuals’ training programs, including observation of staff during implementation.
* * *Maintain current information on all materials, such as emergency packets, individual records, face sheets, etc.
* Programmatic: * Act as an appropriate role model.
* Perform ongoing assessments of the service needs of individuals to include ISPs, quarterly reports and other assessments.
* Counsel individuals and teach them how to define and solve problems by facilitating self- expression and other means of communication.
* Encourage integration of individuals into the community.
* Teach individuals independent living skills, including, but not limited to: money management, food purchase and preparation, personal care, community mobility and resources awareness, daily program/job responsibilities, use of leisure time and interpersonal skills.
* Provide transportation, when necessary, of individuals to work sites, appointments, and activities.
* Advocate human rights and the right to the least restrictive environment.
* Perform other duties as required.
* *Provide assistance and input into the creation of the Individual Service Plan by completing the Feedback Form).
* * *Compile monthly documentation/data sheets and submit them to the Program Manager.
* * *Prepare the necessary paperwork, including medication charts, waiver schedules, daily log sheets, etc.
for the next month, prior to the end of the month before.
* * *Prepare and submit Quarterly Reports, documenting progress of individuals.
* * *Assist with and provide input into preparation of Waiver Plans.
* * *Assist with writing checks for individuals’ accounts to pay bills.
* *HCBS RIGHTS* In addition to the above responsibilities, the staff will also be responsible to the individual they support the following.
* *Individuals will be integrated in and support full access to the greater community.
* * Ensure an individual's rights of privacy, dignity and respect, and freedom from coercion and restraint.
* Optimize, but does not regiment, individual initiative, autonomy, and independence in making life choices, including but not limited to, daily activities, physical environment, and with whom to interact.
* Facilitate individual choice regarding services and support, and who provides them.
Qualifications: * Bachelor's degree in a human services-related field preferred.
* Two or more years’ experience of direct service with adults with intellectual/developmental disabilities/vulnerable populations, preferably in a community-based residential setting.
* One year of supervisory experience required.
* A minimum of 2 years of administrative experience that includes the supervision of staff; budget preparation and maintenance; program development and implementation; quality assurance planning, implementation, and auditing is required.
* Must meet AP and funding jurisdiction current requirements for criminal background/records check.
* A valid health certificate to meet current funding jurisdiction requirements is required.
Physical Qualifications: * Free TB communicable form.
* Physically able to drive a vehicle and eligible for a driver’s license.
* Able to complete household chores.
* Able to climb stairs *and physically maneuver around the home and community.
* * Able to assist individuals with evaluation.
* Insurable under Angelic Place’s automobile insurance policy.
* *Able to communicate effectively with individuals.
* * Ability to become First Aid and CPR certified.
* Ability to lift 50 pounds.
